The importance to health records is that it is easier to treat a patient if you already have the history of the patient illnesses and what medications have helped in the past as well as it will tell them anything they are allergic. They are also important for you don’t have to retake the same test over again if you already have it on file at the hospital you went too. Now with the electronic health care they are heping to make the paients experience better for them with less wait time and questions. Also with the electronic health records it helps the doctor to be reminded of when a new test needs to be done or when a prescription needs to be filled as well. Component of the health record Description Adminerstrative system component This where the patient gets registered, admitted, and discharged this information is very important without this in health records it would make it a lot harder to get the patient seen. This is why it is a key component to health care because without this it wouldn’t run as smoothly. Laboratory system component This is where the doctor can put in orders to get lab work done then someone will bring the sample to the lab to get processed. Then later the doctor can find the results and figure out a diagnosis of the patient or order more labs to get done on the patient. Radiology system component This is where the doctor can order a xray of a patient and then the patient gets sent down to Radiology to get xrayed. Then the radiologist will send the pictures to the doctor after they are processed. The doctor will tell the patient why they are in pain and the next steps for them. Pharmacy system component This is where the doctor can order perscriptions and send them to the pharmacy without writing out a prescription and giving them a paper to bring to pharmacy. Every patient would have a code with them instead of labeling them they just used codes to tell them apart in a hospital. Also easier to keep track of who is who. Risk is defined as the probability that some favorable or unfavorable event will occur. Risk in investments is usually related to the chance that an unfavorable event will occur that will reduce the amount of one’s investment. U.S. government bonds are not completely riskless. Although the risk with a portfolio of government bonds is less risky than other types of portfolios such as long-term corporate bonds; there is still some level of risk with long-term government bonds. The best way to assess the risk on long-term government bonds is to evaluate the standard deviation of the portfolio. The smaller the deviation, the smaller the risk involved (p175). Long-term government bonds carry a standard deviation of 9.4% with an average return of 5.7% (p180). These numbers indicate that in any given year, the rate of return varies from the 5.7% average by positive or negative deviation of 9.4%. Essentially, the rate of return could conceivable be a negative 4.3%, where one would lose money. Therefore, long-term government bonds do carry a lower level of risk compared to other portfolios, but should not be considered riskless. If one had a $250,000 portfolio of 30-day Treasury bills (T-bills) and every 30 days the bills matured and the principle was reinvested in a new batch of bills; and the investor lives on the investment income for a constant standard of living, is this a riskless investment? T-bills are not truly riskless when contained in a rolling portfolio. If one invested in a single one year T-bill, regardless of economy, the standard deviation is zero. Yet, when a portfolio becomes a rolling profile, the investment income will vary depending on what happens to the level of interest rates for each monthly period in this scenario. In a rolling T-bill portfolio, the standard deviation is 3.2% with an average return of 3.9% (p.180). Therefore, stand alone T-bills can be considered riskless, but rolling T-bill portfolios will carry a very small risk of deviating investment income from rollover to rollover.

Essay Question: In order to find out how things really are, one must understand the filters through which one perceives the world. I believe that we perceive and therefore know. We perceive through emotions, language and reason. These are also known as the ways of knowing. Thus, it is appropriate to use the Woolman’s twelve ways of knowing to talk about perception. Reality is what we see, hear, smell, touch, taste and feel. It is what we observe around us using our senses and feelings. However, how do we know that we are really seeing reality and not our perception or something that our brain can accept and process? There are colors that our eyes cannot see for example ultra-violet and infrared-red. These colors are invisible to the human eye therefore we cannot see them. What we perceive is a clean Singapore. How do we actually know that Singapore is not littered with invisible rubbish? Of course, there are special devices that allow us to see these two colors and prove that there is no such rubbish. However, you can never be sure that there are no other colors invisible to our naked eyes. The things that we have always believed to be true, because we have ‘seen’ it with our own eyes may actually be non-existent so can we trust our eyes? Is what we see real? ‘My desk is in the study room’. This statement’s so called ‘proof’ is based on ‘unproven’ premises and therefore nothing is true and reality is not real. This is skepticism. When can we be sure? We can only be sure when we begin with the simplest, most basic premises which is certainly impossible to doubt. This can be summed up into these three lines ‘what we see is not real’, ‘you cannot be sure whether you are dreaming or not’, ‘there is only thing that you can be certain of and that is your consciousness of your own existence’. Xenophanes once said that we can always learn more than we know, but we can never be sure that we have reached any final truth. Thus is seeing really believing or believing is really seeing? If your father brought you up teaching you that drugs is good for you, you will the perception that drugs is good and take drugs without it pricking your conscience, eventually fall into addiction of drugs. Therefore, are you able to trust your conscience or the persons who brought you up and helped program your conscience? Dreams seem real and convincing. Is it possible that you remember your dreams as you remember past experiences? This seems absurd, but sometimes you can mistake a dream for reality (dreams often seem very real), as a result you may also mistakenly store that reality-look-alike thought in your brain. You mistakenly made these memories and you can no longer be confine your memory to real-life experiences, which in itself are not very convincing and have its areas for skepticism, but fake and pure materials from your imagination. Every argument or proof proceeded from premises, which it did not itself establish. Trying to demonstrate the truth of those premises by other arguments or proofs had to be based on undemonstrated premises. Therefore, no ultimate ground of certainty could ever be reached. What a valid argument proves is that its conclusions follow from its premises, but it is not at all the same as proving that those conclusions are true. Every valid argument starts with an ‘if’: if ‘p’ is true then ‘q’ must be true. That leaves open the question of whether or not ‘p’ is true. The argument itself cannot prove that because it has already assumed it and to have assumed already what sets are out to prove would be to move in a vicious circle. Perception can be simplified into two basic questions. Is your perception true? Why is it true or false? Then you can ask yourself how do you perceive? We know through our five senses? These senses send impulses or messages to our brain from the outside word. The brain then interprets and creates the real world in our brain. How does the brain interpret the impulses or messages? It can be done through past experiences, social, cultural, religious communities, spatial familiarity for example patterns and shapes, biological limitations, existing learning structures, language and self perception such as how we think or what we believe. It is indeed doubtful whether anyone could live based on complete perception, or if they could, whether such a life would be worth living. However, this refutation of perception, if refutation it is, is not a logical argument. In practical life, we must steer a middle course between demanding a degree of certainty that we can never have and treating all possibilities as if they were of equal weight when they are not. Man possesses an inherent selfishness which causes him to strive constantly for self-preservation, and in turn is the cause of his suffering (or seeking) competition, glory, and distrust. Such a state is ultimately â€Å"no society; and which is worst of all, continual fear and danger of violent death† (1996: xiii). This is not to say that man is a blundering, blind fool simply seeking to grab that which promises to bring greater comfort and success to his life. Rather, man is in possession of reason, which causes him to grasp an understanding of right and wrong conduct. Yet, because no formal standards of right and wrong exist in a state of nature, opinions and rights clash and differ. This is not to assume that Hobbes denies the universality of morality or natural law, rather man is governed by agreements and contracts. However, Hobbes’ contracts are a product of the selfishness of man, and hence are not based upon any form of honour or trust because they will be valid to the point that an individual believes that another will not fall foul of his promise. An example would be that Y does not punch Z because Y does not want Z to punch him. This ‘contract’ is formed on pure selfishness, and only extends to the point that Z complies with the agreement. If Y feels that Z’s agreement lacks strength, he will quickly feel free to break his part of the contract. Such contracts, because they are without honour and because they are a product of selfishness, are very likely to be breached. If we are to apply these points on an international scale, some contradictions emerge. While on the surface it would appear that each state has the right to do anything, the existence of equality is highly questionable. Hobbes evidently thinks that a â€Å"genuine condition of war† exists between states (Hokestra 2007: 118), though not their individuals; rather their sovereigns who are constantly â€Å"in the state and posture of gladiators† (1996: xiii, 12, 63/78). The lack of common power on an international level today is evident, yet could this be utilised to lead to the conclusion that each state is constantly read for, or under threat of war? The temptation to answer this query negatively is backed by the concept of equality. Indeed, there is a great deal of â€Å"radical uncertainty† surrounding the cooperation between states (Newey 2008: 161). Though Hobbes saw men as equal in a state of nature, it could not be said that all states are equal; the opposite is actually evident. America certainly does not feel the need to harbour pre-emptive aggres sion against countries such as Iceland, for example. This leads to the conclusion that internationally, states are in a state of war as man is in the state of nature (Bull 1977: 49). This concept can also be applied to Hobbes’ view of man in nature as essentially unsociable: states across the globe often enter into mutually beneficial agreements. Even larger states provide aid to third world countries, particularly after crises and where poverty is extreme. Although these distinctions may be rather primitive, they gather much ground in establishing weaknesses in Hobbes’ theory being applied on an international level.
